FollowMyHealth™ is a useful tool delivering anywhere, anytime access to your health records, and will enable you to take a proactive role in managing your health care.

All patients must first register at the clinic. Once you have provided your name, address, phone number and email, you will be registered and provided with a password. You will then access the portal through vmchealth.com, as a drop-down under the “For Patients” link on the home page.

Once you are signed up with FollowMyHealth™, you will be empowered to:

• Request an appointment
• Check on upcoming appointments
• Review your medical records
• Quickly access lab results
• Read your doctor visit summary
• Update your health information
• Refill your prescriptions
• Check your vital signs (such as weight and blood pressure)
• Pay your bills

You can also download the portal app to your smart phone and set appointment reminders.
If you are not comfortable with the online system, you may identify someone in your family to review and update the information and request appointments on your behalf.
