The Wellness Center is a no-cost program designed just for KHMG’s Medicare patients to help ensure that they receive their free annual preventative care visits.
The goal is to keep patients healthier and more independent. Kaweah Health Medical Group launched The Wellness Center to ensure that our Medicare patients can take full advantage of the preventative health services that Medicare provides at no cost.
Each Wellness Center patient receives a personalized Health Risk Assessment that identifies risk factors for future health problems. Staff then develops a screening schedule to monitor each patient’s health going forward, and offers education on health conditions and medications.
